| Description: | SynCare is currently searching for a Licensed Social Worker to help us expand our disease and case management programs. This position requires an individual that has a passion for providing counseling, education and services to assure healthy outcomes take place in a healthy and safe environment.
SynCare's programs provide:
1. OB Case Management - to high risk pregnancies;
2. Well Child Care - as we follow the children through age 2;
3. Hospital to Home Support - for children and women who have been hospitalized with respiratory related emergencies.

| Duties: | You will provide social services as part of a team to help individual members needing OB Case Management; Well Child; and Hospital to Home Support.
You will utilize telephonic case management and home visits to help individuals to appropriately utilize services. This is done by providing appropriate health education and encouraging behavior modification. Social Services are a major part of our programs.
Our services target high users of health care services and those that are high risk and/or with chronic diseases. We focus on prevention, education and behavioral change. You will work closely with our team of RN Case Managers.
We are looking for an individual to help start our programs and act in a leadership role over additional Social Workers as our programs expand.

| Qualifications: | Licensed Social Worker in the state of Missouri;
Masters Degree Required
Medical Social Work Experience Prefered |
