| Description: | The Perinatal Care Coordinaton Registered Nurse Case Manager will provide case management and education to members enrolled in the Perinatal Care Coordination Program. The RN Case Manager will be a key component in achieving the overall goal of enhancing clinical outcomes, reducing member/baby hospitalizations, and reducing healthcare costs to clients. |
| Duties: | Overall Responsibilities:
-Conduct home visitations for high risk perinatal memebers.
-Manage the assigned member caseload according to policy and procedure.
-Conduct telephonic coaching and follow-up services.
-Perform timely and accurate re-assessments.
-Assist with problem identification, goal setting, decision-making, problem-solving, and addressing priority needs.
-Communicate effectively with OB provider for approval of member-centered plan of care.
-Maintain current member records in the data management system.
-Communicate effectively with all members of the Perinatal Care Coordination Team.
-Accept related assignments as directed by the supervisor.
-Travel through the western part of southern Indiana required.

| Qualifications: | Must possess a Baccalaureate/Associate Degree of Nursing from an accredited college or university.
Must possess or be able to obtain a valid Indiana RN license.
At least one year of strong clinical experience.
OB/L&D/Perinatal experience preferred.
Excellent verbal and interpersonal communication skills.
Possess knowledge of problem identification and problem solving processes.
Must have reliable transportation and auto insurance.
