| Description: | SynCare LLC is currently recruiting for a part time Registered Nurse to provide case management and education to members enrolled in the Perinatal Care Coordination Program in a 12 county region. This position will require you to work approximately 2 to 3 days per week. You will have a reasonable amount of flexibility to determine your schedule. |
| Duties: | In this position, you will conduct home visits with high risk perinatal members in your assigned case area. Responsibilities include education, coaching, and providing follow-up services. You will maintain member records in our electronic medical records system and communicate effectively with providers and other members of the team. |
| Qualifications: | Candidates must have either a associated degree or BSN form an accredited college or university and must possess or be able to obtain a valid Indiana RN license. Excellent communications and computer skills are required. You must have a valid drivers license, reliable transportation and proof of auto insurance. |
